Job summary

Symbii Home Health & Hospice in Chubbuck, Idaho is seeking an on-call Registered Nurse (RN) to provide high-quality home health and hospice care. The RN will perform assessments, develop and adjust care plans, and administer treatments in collaboration with physicians and caregivers.

This full-time role involves on-call duties and coordination with a multidisciplinary team. The ideal candidate holds an Idaho RN license, a valid driver's license, and preferably Home Health/Hospice experience.

Qualifications

• Current Idaho RN license.
• Valid driver's license.
• Home Health and Hospice experience preferred.

Responsibilities

• Completes initial patient assessment and develops a care plan with patient/family input.
• Monitors patient status and re-evaluates nursing needs regularly.
• Administers medications and treatments as prescribed by the physician.
• Coordinates discharge planning and coordinates patient care with the clinical team.
• Communicates with primary physician and other care providers to ensure continuity of care.
