Job description

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based settings to help patients and their families ease the burden of managing a life-limiting illness or end-of-life care.

Responsibilities include:

  1. Providing on-call patient visits, scheduled or unscheduled, for urgent physical and psycho-social issues during after-hours shifts, in coordination with the Call Center Triage and On-Call Manager.
  2. Responding via cell phone to Triage visit requests and communicating effectively with the on-call team, including Hospice physicians, to address urgent patient needs and establish a safe plan of care.
  3. Implementing the plan of care and educating patients and primary caregivers on changes, including medication management, patient safety, and symptom management.
  4. Attending patient deaths as directed to provide emotional support and guidance to families.
  5. Reporting to triage and the primary Hospice team regarding patient visits, changes to the plan of care, and follow-up needs.
  6. Following NYS Standards for Professional Registered Nurses.

This is an Office Field position, primarily home care, nursing, RN, hospice, and related roles.

Qualifications include:

  1. Graduation from an accredited School of Nursing.
  2. At least two years of experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing preferred.
  3. Valid New York State RN licensure; a valid NYS driver’s license is preferred.
  4. Eligibility to sit for the national board certification CHPN within one year of hire.
  5. Proficiency in Word, Excel, and Windows applications.
