We offer a broad range of services in the community or facility-based settings to help patients and their families ease the burden of managing a life-limiting illness or end-of-life care.
Responsibilities:
- Provide on-call patient visits, scheduled or unscheduled, for urgent physical and psycho-social issues during after-hours shifts, in coordination with the Call Center Triage and On-Call Manager.
- Respond via cell phone to Triage visit requests and communicate effectively with the on-call team, including Hospice physicians, to address urgent patient needs and establish a safe plan of care.
- Implement the care plan, educate patients and primary caregivers on changes, medication management, patient safety, and symptom management.
- Attend patient deaths to provide emotional support and guidance to families.
- Report to triage and the primary Hospice team regarding patient visits, changes to the care plan, and follow-up needs.
- Follow NYS Standards for Professional Registered Nurses.
Position Details:
This is an Office Field position, primarily home care.
Qualifications:
- Graduate from an accredited School of Nursing.
- At least two years of experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing preferred.
- Current New York State RN licensure; a valid driver’s license is preferred.
- Will sit for the national board certification (CHPN) within one year of hire.
- Proficiency in Word, Excel, and Windows applications.
