Job summary

Hearth Hospice is seeking a Full Time On-Call Registered Nurse (RN) to join our team, delivering compassionate care to hospice patients and families during critical moments. As an On-Call RN, you will identify needs, coordinate care, and provide preventive measures within patients' homes and our agency.

Qualifications

• RN license in good standing with no encumbrances.
• Minimum one year RN experience in medical-surgical/acute care; hospice a plus.
• Proficient computer skills and EMR experience.
• CPR certification if required by state.

• Be the Lifeline: Respond to a variety of patient needs, from urgent to routine, your unwavering support will make a difference in critical moments.
• Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
• Maintain Accurate Documentation: Every detail is crucial in delivering high-quality care, both in the patient's home and within our agency and regulatory standards.
• Provide Support for Families: Guide and support families in caring for their terminally ill loved ones.

Qualifications

• Bachelor's degree in nursing or health care preferred.
• Hold current unencumbered license as a Registered Nurse (RN).
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a Plus!
• Must be computer proficient in typing and various programs, including a background in EMR.
• Possess and maintain current CPR certification if required by state.

$73,000 - $83,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate's relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
