Job Summary

The Hospice On-Call Nurse responds to calls from patients, families, caregivers, and referral sources. The nurse assesses, utilizes critical thinking, and gives advice or delivers care to patients using the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.

Responsibilities
  • Be available by phone or pager to meet the needs of patients during assigned shifts.
  • Identify and coordinate patient/family care to support patients with advanced illness and families in home, skilled nursing facility, or residential care facility.
  • Document and communicate on‑call interventions, care plan changes, and physician contacts with the Case Manager.
  • Use teaching, assessment, and intervention skills to provide comfort care and maximize the quality of life for patients and families.
  • Assess hospice needs, gather data on social, economic, and cultural factors that may influence health and well‑being.
  • Respond to calls from the answering service and voicemail system promptly.
  • Triage patient‑related calls, determine the needs of the caller and respond appropriately by phone or with a visit.
  • Implement changes to the plan of care as needed with interdisciplinary collaboration.
  • Document all calls, interventions, and care plan changes in a manner consistent with policy and procedure.
  • Assist patients, family members or caregivers with concerns and show empathy, respect confidentiality and privacy, and communicate in a courteous and respectful manner.
  • Coordinate services with contracted vendors, outside agencies and interdisciplinary team as needed.
  • Investigate and follow through on orders or requests for service or information.
  • Attend patient deaths to provide support to the patient’s family, contact the mortuary, advise on disposal of narcotics, and notify the IDT as well as contracted vendors providing services to the patient.
  • Provide bereavement resources to the family as appropriate.
  • Respond to referral calls, provide the caller with information regarding hospice care, screen calls for appropriateness, take patient information and coordinate with the admin on call to schedule the visit.
  • Perform patient admission visits and other duties within scope of practice as assigned by supervisor.
  • Participate in the agency’s on‑call rotation as prescribed by the needs of the agency to provide nursing service to clients when required outside of regular office hours.
  • Provide point‑of‑service electronic charting and documentation at time of patient visit or admission.
  • Operate a computer or tablet PC, and proficiency with Windows‑based software programs.
Qualifications
  • Education: Graduate of an accredited school of nursing.
  • Licensure: Valid registered nurse license; current Basic Life Support certification; valid state driver’s license.
  • Experience: Minimum one year of recent professional nursing experience; hospice or home health experience preferred.
  • Knowledge and Skills:
    • Operate within policies, procedures, state and federal regulations, and patient plans of care.
    • Manage and care for patients after business hours.
    • Cope with emotional stress and tolerate individual lifestyles.
    • Show sensitivity to needs of patients with advanced illness and families, and to personal feelings about dying and death.
    • Work independently and demonstrate good interpersonal and computer skills (Microsoft Word, email).
  • Transportation: Reliable transportation, valid driver’s license, and auto insurance; staff required to travel to patient homes.
