Job summary

CODA in Beaverton, OR seeks an On-Call Recovery Mentor to provide and coordinate recovery support services in the community, reinforcing treatment goals and accessing resources. This on-call role covers call-outs and vacation time as needed in sobering.

The position requires a high school diploma or GED. You will assist participants with access to shelters, medical and social services, and document contacts in electronic health records.

Qualifications

• High school diploma or GED.
• CRM certification at time of hire.
• BLS certification or ability to acquire within 90 days.
• Crisis Prevention training within 90 days.
• Valid Oregon or Washington driver’s license.

Responsibilities

• Assist participants in accessing shelters, medical and social resources.
• Assist participant with resources and referrals based on needs.
• Complete needs assessment and SUD triage tool.
• Assist individual with meeting sobering goals.
• Facilitate connection to the recovery community and supports.
• Teach and model recovery behaviors with professional boundaries.
• Facilitate peer recovery and life skills groups if needed in withdrawal management.
• Build and maintain positive relationships with recovery community and organizations.
• Document contacts in electronic health records and care systems.
• Promote community integration in housing, employment, education, family and social support.
• Assist in tracking data elements for program reporting.
• Accompany and may drive patients to recovery related appointments.
