Momentum is seeking a PD Peer Navigator for a temporary, on-call role in San Jose, CA. The position supports clients and families, requires peer experience, and offers a 3-month assignment with potential certification completion.

The role is 100% field-based, with pay at $29.14 per hour plus shift differentials for overnight, evening, and weekend shifts. Spanish, Vietnamese, or Russian speakers are preferred.

Qualifications

• High School diploma, GED, or equivalent education.
• Personal experience in a mental health setting preferred.
• Experience as a mental health consumer or family member required.

Responsibilities

• Assist clients in navigation of the mental health system.
• Establish links with service providers and educational institutions.
• Advocate for clients and coordinate with multidisciplinary teams.
• Provide outreach and client support in the community.
