Job summary

Telecare Sonoma ACT seeks a Peer Health Navigator to engage members with mental health services, support goal setting and self-management, and promote Telecare's Recovery Culture in a recovery-oriented program.

Shifts include full-time daytime hours; wage range starts around $23.15–$25.38 per hour, with geographic differentials applied to pay according to location. Requires relevant peer certification and background checks.

Qualifications

• Requires high school diploma or GED.
• Must hold California Medi-Cal Peer Support Specialist Certification or obtain within 120 days.
• CPR, First Aid and CPI certification required at hire or prior to providing care.

Responsibilities

• Engage and connect members to mental health, primary care and related services.
• Help members develop personal health goals and advocate for themselves.
• Support follow-up care plans and self-management skills.
• Respond to emergencies with de-escalation and ensure least restrictive environment.

Skills

Two years experience

CPR

First Aid

CPI certified

Driver's license

Willingness to use personal vehicle

Education
