Mission Nursing Home in Plymouth, MN is seeking a Nurse Aide Registrar (NAR) to provide routine daily nursing care under the supervision of the Director of Nursing, following established procedures and resident care plans.

Responsibilities include assisting residents with personal care, monitoring vital signs, documenting intake and output, reporting changes to the nurse, and supporting restorative programs while ensuring safety and confidentiality.

Qualifications

• Completion of approved Nursing Assistant training program.
• Successful completion of written and skills exam and in good standing with Minnesota Nursing Assistant Registry.
• Must pass Minnesota DHS background check and OIG exclusion search.
• Excellent oral and written communication skills in English.
• Friendly, professional bedside manner and patient-focused approach.

Responsabilités

• Assist residents with personal care (bathing, dressing, grooming, toileting) per care plan.
• Follow written plan of care and document observations and interventions.
• Monitor vital signs, weights, intake/output as assigned and document accurately.
• Ensure safety, cleanliness, dignity, and confidentiality for residents and report changes to the nurse.
