Job summary

The Providence Willamette Falls Medical Center in Oregon City is seeking a licensed Clinical Social Worker (LCSW) for social work services on-call during day shifts. Qualifications

• Master's Degree in Social Work from CSWE-accredited program.

• At least 2 years of relevant social work experience (CSWA experience allowed).

Responsibilities

• Conduct biopsychosocial/spiritual assessments and diagnoses.
• Develop person-centered care plans with the IDT.
• Collaborate with inpatient and outpatient teams to coordinate care.
