Job summary

Vitality Hospice is seeking an On-Call Registered Nurse (RN) for full-time after-hours hospice care across New Jersey. You will respond to patient needs, manage symptoms, support families, and coordinate with an interdisciplinary team to ensure comfort during evenings and weekends.

The role emphasizes compassionate bedside care, strong assessment skills, and the ability to work independently while collaborating with clinicians. A valid NJ RN license and driving capability are required.

Qualifications

• Current New Jersey RN license.
• Minimum 1 year RN experience.
• Hospice experience preferred; training offered.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and compassionate bedside manner.
• Ability to work independently while collaborating with the clinical team.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Respond to after-hours patient needs and symptom management.
• Perform nursing assessments and develop appropriate interventions.
• Provide compassionate education and emotional support to patients and families.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Complete timely, accurate documentation in the EMR.
• Maintain compliance with Medicare, state regulations, and hospice standards.
• Participate in rotating on-call coverage while providing exceptional customer service.

Position Overview

As an On-Call Registered Nurse , you'll provide compassionate after-hours hospice care, responding to patient needs, managing symptoms, supporting families, and ensuring comfort during evenings, nights, and weekends. You'll work alongside an experienced interdisciplinary team dedicated to delivering exceptional hospice care.

Schedule

Full-Time On-Call

• 48-hour on-call weekend shift
• Four weekday on-call shifts (Monday-Friday | 5:0 PM - 8:00 AM)

What You'll Do

• Respond to after-hours patient needs and symptom management
• Perform nursing assessments and develop appropriate interventions
• Provide compassionate education and emotional support to patients and families
• Coordinate care with physicians and the interdisciplinary hospice team
• Complete timely, accurate documentation in the EMR
• Maintain compliance with Medicare, state regulations, and hospice standards
• Participate in rotating on-call coverage while providing exceptional customer service

What We're Looking For

• Current New Jersey Registered Nurse (RN) License required
• Minimum of 1 year of RN experience (Hospice, Home Health, Oncology, ICU, ER, or Med-Surg experience preferred)
• Hospice experience preferred, but we're willing to train the right candidate
• Strong clinical assessment and critical thinking skills
• Excellent communication and compassionate bedside manner
• Ability to work independently while collaborating with the clinical team
• Valid driver's license, reliable transportation, and current auto insurance
