Job summary

Grace Healthcare Services is seeking an On Call Visit Registered Nurse in Essex/Morris Counties. This full-time role runs Monday–Friday, 5pm–8am, delivering compassionate hospice care with strong collaboration to the RN Case Manager and IDT.

You will provide in-home hospice visits, ensure accurate documentation, and support families through challenging moments with dignity and professionalism.

• Act as a vital member of the Interdisciplinary Team (IDT), contributing to a comprehensive and responsive plan of care.
• Work under the supervision of a Registered Nurse to assess and meet the multifaceted needs of patients and their families.
• Provide continuous, compassionate care, ensuring adherence to hospice care standards and protocols.
