Qualifications

• Graduate of an approved school of professional nursing.
• Current, active license in the state of Arizona Board of Nursing.
• Current CPR, health clearance, TB clearance.

Responsibilities

• Apply admission and recertification criteria to determine eligibility and care level.
• Assess illness impact on physical, functional, environmental, safety and psycho-social needs.
• Assess pain and develop an individualized pain management plan.
• Identify patient verbal and non-verbal communication patterns.
• Assess caregiver ability to meet patient needs at start and throughout care.
• Develop an individualized palliative plan of care addressing discomfort and relief.
• Additional duties per the job description.
