Job summary

Affinity Hospice Management LLC seeks an experienced on-call RN to coordinate and deliver compassionate hospice care in patients' homes and in facility settings around Cincinnati, OH.

You will support families, manage comfort and symptom relief, document care, and coordinate with the interdisciplinary team. Weekend, night, and on-call shifts apply; driving to patient locations required and CPR/Ohio RN licensure are mandatory.

Qualifications

• RN license (Ohio) required.
• CPR certification required.
• Valid driver's license and current auto insurance coverage.
• Graduate of an accredited school of nursing.

Responsibilities

• Coordinate care and provide direct care/support for hospice patients and families in home and facility settings.
• Manage comfort, symptoms, medications, and documentation; liaise with hospice interdisciplinary team and facilities.
• On-call support after agency hours; weekend availability.
