Job summary

Capital City Hospice is seeking an on-call Registered Nurse (RN) to triage after-hours calls in Columbus, OH and surrounding areas. This role involves assessing needs, coordinating care, and updating care plans with the interdisciplinary team.

You will obtain data on patient, family, and spiritual factors, communicate with physicians, and provide death pronouncement support while maintaining clinical records. Colleague support and compassionate care are core priorities.

Qualifications

• Graduate from an accredited registered nursing program.
• Possession of a current license to practice in the state as a Registered Nurse (OH).
• Minimum of two years of varied work experience as a professional nurse.
• Must possess current CPR Certification.

Responsibilities

• Assume responsibility for triage of after hours patient and family calls, determining the need for services, assessments, symptom management and care plan updates.
• Obtain data on physical, psychological, social and spiritual factors affecting patient and family needs; initiate intervention by the interdisciplinary team.
• Initiate communication with the attending physician and other team members for coordinating optimal care.
• Perform clinical pronouncement of death and provide support to family; maintain accurate clinical records and care plans.
