St. Croix Hospice in Brighton, MI is seeking a Registered Nurse On-Call to support our 24/7 care model, providing after-hours care during evenings, nights, and weekends. This role follows a 7 days on/7 days off overnight schedule, offering flexibility and meaningful impact while serving patients in the Lansing area with office-based work in Brighton.

Meetings may be attended remotely. Candidates must be a licensed RN with acute care and hospice/home care experience, and possess CPR certification

Qualifications

• Graduate of an accredited nursing program (Associate's, Bachelor's, or Master's).
• 1+ years of recent acute care experience; hospice or home care preferred.
• Current RN license and CPR certification.
• Respond to triage calls and provide after-hours nursing visits.
• Develop and adjust individualized care plans.
• Administer medications and teach patients and caregivers.
• Maintain timely clinical documentation.
• Must have reliable transportation and valid driver's license.

• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates.
• Develop and adjust individualized care plans based on evolving needs.
• Administer medications and treatments as prescribed, offering education and support to patients and caregivers.
• Maintain timely and accurate clinical documentation of care, outcomes, and communication.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.
