Job summary

Heartland Hospice seeks an On-Call Registered Nurse to provide compassionate hospice care to patients in homes and facilities. The RN triages after-hours calls, makes urgent visits, assesses symptoms, and implements care plans focused on comfort and dignity.

The nurse collaborates with an interdisciplinary team, communicates status changes to providers, and supports emotional and spiritual needs, honoring patient goals and cultural values while maintaining regulatory and quality standards.

Qualifications

• RN licensure and active nursing license in the applicable state.
• Experience in hospice or palliative care preferred.
• Ability to educate patients/families and coordinate with an interdisciplinary team.

Responsibilities

• Triage after-hours calls and provide urgent home or facility visits.
• Assess symptoms and adjust care plans for comfort and dignity.
• Administer medications and manage pain and other symptoms.
• Document care activities promptly and accurately.
