Job summary

Heartland Hospice seeks an On-Call Registered Nurse to provide compassionate hospice care to patients with life-limiting illnesses in homes and facilities. The RN triages after-hours calls, makes urgent visits, assesses symptoms, and implements care plans focused on comfort and dignity.

The nurse collaborates with an interdisciplinary team, communicates status changes to providers, and supports emotional and spiritual needs, honoring patient goals and cultural values while maintaining regulatory

Qualifications

• RN license in good standing.
• Experience in hospice or palliative care is preferred.
• Strong clinical assessment and communication skills.

Responsibilities

• Triage after-hours calls and determine urgent patient needs.
• Perform urgent home/facility visits and assess symptoms.
• Develop and adjust care plans focused on comfort and dignity.
• Administer medications and document patient status promptly.
