Silverado Hospice SouthHouston is seeking an RN On-Call to provide after-hours coverage for our Houston-area patients and families. As part of a nationally recognized team, you will perform nursing evaluations, coordinate care, and document visits promptly to ensure seamless continuity of care.

Qualifikationen

• Registered Nurse with a valid state license.
• Minimum of 1 year of nursing experience.
• Hospice experience preferred but not required.
• Valid driver's license with good driving record and reliable transportation.
• Ability to lift 50 pounds.
• Must clear a background check, physical and drug screening.
• Flexibility to work outside regular hours based on business needs.
• Comfort working in environments with animals and pets.

Aufgaben

• Perform comprehensive nursing evaluations and after-hours assessments for patients with changes in condition.
• Assess signs of discomfort, including pain, and take prompt action.
• Initiate and update plans of care; provide nursing follow-up and caregiver instruction.
• Submit accurate documentation within 24 hours of visits.
• Coordinate services across settings to ensure quality and appropriateness of care.
• Follow approved care plans, modify as needed, and communicate with physicians.
