Vitality Hospice & Palliative Care is seeking a compassionate Registered Nurse (RN) for a full-time 7 Days On / 7 Days Off On-Call position in Pennsylvania. The role offers a predictable schedule with every other week completely off, while providing meaningful hospice care to patients and families.

You’ll work independently in the field, supported by an experienced team, with comprehensive benefits and a focus on quality patient outcomes.

Qualifications

• Current Pennsylvania Registered Nurse (RN) license necessary.
• Minimum 1 year of RN experience required.
• Hospice, Home Health, Oncology, ICU, ER or Med-Surg experience preferred.
• Hospice experience preferred but training can be provided for the right RN.
• Strong assessment and critical-thinking skills emphasized.
• Compassionate and confident communication style.
• Ability to work independently and make sound clinical decisions.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to after-hours patient needs and changes in condition.
• Provide symptom and pain management.
• Perform nursing assessments and determine interventions.
• Make patient visits when clinically necessary.
• Educate and support patients and families.
• Coordinate care with physicians and the hospice team.
• Complete timely and accurate EMR documentation.
• Maintain continuity of care during the on-call week.
• Ensure compliance with Medicare, state, and hospice standards.

What You'll Do

As our On-Call RN, you'll provide compassionate after-hours hospice care and be a reassuring presence for patients and families when they need support most.

Your responsibilities will include:

• Responding to after-hours patient needs and changes in condition
• Providing symptom and pain management
• Performing nursing assessments and determining appropriate interventions
• Making patient visits when clinically necessary
• Educating and supporting patients and families
• Coordinating care with physicians and the interdisciplinary hospice team
• Completing timely and accurate EMR documentation
• Ensuring continuity of care throughout your on-call week
• Maintaining compliance with Medicare, state, and hospice standards
