Job summary

Phoenix Home Care & Hospice is seeking an On-Call Registered Nurse to support our Springfield hospice team. This role delivers timely, skilled end-of-life care in patients' homes, especially during nights and weekends.

Must have Missouri LPN or Compact license with hospice experience preferred. You’ll work autonomously, document care, and collaborate with physicians and the hospice team in a compassionate, patient-centered environment.

Qualifications

• Missouri LPN or Compact License accepted.
• Hospice nursing experience preferred.
• Able to work independently in home settings.

Responsibilities

• Respond to urgent hospice needs during nights and weekends.
• Complete routine and as-needed visits in home settings.
• Triage urgent situations and collaborate with physicians.
• Document accurately and maintain regulatory standards.
• Engage with the interdisciplinary hospice team and families.
