Job summary

HeartShare is seeking on-call Direct Support Professionals to support residents in person-centered services. The role requires advocating for individuals, implementing goals, and ensuring well-being, with at times required to assist with personal care, mobility and community participation.

Initial offers begin at $19 per hour with additional compensation possible after hire for certifications, training, and performance.

Qualifications

• High school diploma, GED, CNA or Direct Support Professional certificate required.
• One year or more work experience with individuals with developmental disabilities preferred.
• Excellent written, communication and mathematical skills.
• Valid NYS driver license preferred but not required.
• Flexible work schedule.

Responsibilities

• Under supervision, provide support and services advocating for individuals and guiding personal choice and community integration.
• Assist individuals with daily living activities, hygiene, dining, dressing and mobility; promote a safe environment.
• Collaborate with staff to implement care plans; may administer medications per instructions.
• Act to ensure health and safety in emergencies.
• Maintain AMAP, CPR, First Aid and SCIP certifications as required.
• Stand for extended periods and assist with lifts and transfers.
• Promote inclusion and foster friendships within the community.
• Assist in transportation for individuals as needed.
• Maintain positive communication with family, advocates and providers.
• Perform other duties as assigned and compliant with policies.
