Looking for an on-call caregiver to service the North Side of Chicago. Must reside close to the office (zip code 60640).

Caregiver requirements include:

1. Personal care duties
2. Transfers using Hoyer lift / gait belts
3. Providing companionship
4. Dementia experience (preferred but not mandatory)
5. Parkinson's experience (preferred but not mandatory)
6. Reliable transportation
