Job summary

St. Croix Hospice seeks an RN on-call for Minnetonka, MN. This full-time role involves after-hours triage, symptom management, care plan updates, and patient education.

You will administer medications, develop individualized plans, and document outcomes while maintaining reliable transportation and an active RN license with CPR certification. Qualifications

• Graduate of an accredited nursing program (Associates, Bachelors, or Masters)
• 1+ years of recent acute care experience required; hospice or home care experience preferred
• Current RN license in state of practice and CPR certification
• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates
• Develop and adjust individualized care plans based on evolving needs
• Administer medications and treatments as prescribed, offering education and support to patients and caregivers
• Maintain timely and accurate clinical documentation of care, outcomes, and communication
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

The starting salary range for this role is $88,000-$98,000 annual base salary.An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
