Job summary

Easterseals Louisiana is seeking Support Coordinators to plan, organize, and monitor in-home and community-based services for the OIDD population. You will manage a caseload of about 35-45 participants, with field visits about 50% of time.

The role requires agility to adapt to evolving needs, completion of required paperwork, and participation in training. The position is hybrid, with initial 90 days in-office full-time, plus a background check and drug screening.

Qualifications

• The candidate must hold a Bachelor's or Master's Degree in Social Work from a CSWE-accredited program and have two years of paid post-degree experience providing coordination services.
• A Bachelor's or Master's Degree in Nursing (RN) licensed in Louisiana, with two years of paid post-degree experience.
• A Bachelor's or Master's Degree in a Human Service related field and two years of paid post-degree experience in coordination services.
• A Bachelor's Degree in Liberal Arts/General Studies with 16+ hours in one of psychology, education, counseling, social services, etc., plus two years of post-degree experience.

Responsibilities

• Complete quarterly visits with observations, generate units, and maintain monthly contacts.
• Conduct and complete annual CPOCs according to funder timelines.
• Plan, schedule, and coordinate quarterly meetings with participants and providers.
• Respond to participant changes and complete necessary paperwork.
• Research, access, and link services for participants.
• Communicate with providers to ensure appropriate service provision.
• Maintain all responsibilities regarding critical incidents.
