Job summary

Lorian Health seeks a licensed Physical Therapist to provide home health therapy in San Diego. You will follow physician orders, contribute to Plan of Care updates, and assess patient function to guide care.

The position features a pay-per-visit model ($105–$150), a Monday–Friday schedule with weekend rotations, and comprehensive benefits including a company vehicle and 401(k).

Qualifications

• Minimum of one year of recent experience in a Home Health or Hospice setting.
• Graduate of a physical therapy school approved by the AMA or equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Ability to read and follow written instructions and document care.
• Self-directed with the ability to work with little direct supervision.
• Current CPR certification.
• Licensed driver with an automobile insured in accordance with company requirements.
• Ability to verbally communicate in the language of the client and document all client-related activity.

Responsibilities

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Assisting in developing and updating the plan of treatment or Plan of Care (POC).
• Assisting the physician in evaluating level of function.
• Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
• Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient’s therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making appropriate recommendations.
• Participating in in-service education programs.

• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Has the ability to read and follow written instructions and document care given.
• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.

This position is based on a pay per visit compensation model. Pay per visit ranges from $105 to $150 visit commensurate with experience and depending on type of visit.
