Job summary

Three-Meadows-Post-Acute in Perrysburg, OH is seeking a dedicated Occupational Therapy Assistant to work alongside our staff OT delivering hands-on, resident-centered care.

You will provide direct treatment under supervision, educate families on home programs, assistive equipment, and fall prevention, and help train aides while contributing to care conferences and program quality.

Qualifications

• Associate degree from an accredited COTA program.
• Active state licensure as an Occupational Therapy Assistant in good standing.
• CPR certification as required by state licensure.
• A steady, compassionate presence with residents, families, and colleagues.

Responsibilities

• Deliver hands-on care alongside the Staff Occupational Therapist.
• Provide direct resident treatment under the guidance of the OT.
• Build real relationships with residents, families, and the care team — communicate progress, goals, and discharge planning.
• Educate residents and families on home exercise programs, adaptive equipment, and fall prevention.
• Recommend equipment to restore independence — splints, assistive devices, grab bars, etc.
• Help train aides and provide in-services to fellow staff.
• Participate in Resident Care conferences and contribute to program development and quality initiatives.
