Evolve Therapy Services is seeking a compassionate Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Manitowoc, WI. Part-time or PRN positions available, involving direct patient care and collaboration with an interdisciplinary team.

Responsibilities include planning, implementing, and documenting therapy interventions aligned with the plan of care, regular resident screenings, and ensuring CMS documentation standards are met. Active OTA licensure is required.

Qualifications

• MUST have active OTA licensure in state(s) of practice.
• Associate's degree in occupational therapy.
• Ongoing continuing education and professional development to maintain licensure and certification.

• Plan, organize, develop, and direct Occupational Therapy Services in the facility.
• Conduct regular screening of residents to determine the best intervention for each patient.
• Perform treatment plans and follow short and long-term goals per the plan of care.
• Interpret and communicate evaluation findings and recommendations to residents, families, and team members.
• Establish resident therapy schedules with team members and document therapy services.
• Comply with CMS documentation standards and billing requirements.

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short and long term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications
