Responsibilities

• Maintain patient records by tracking goals and progress.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per regulatory requirements.
• Provide consultation to patients, families, and caregivers regarding physical disorders.
• Communicate progress and plans with supervisor and health team members.
• Provide discharge summaries and document required information in the EHR.
• Supervise rehabilitation aides and/or students when applicable.
