Job summary

INNOVATE REHAB AND WELLNESS in St. Louis, MO is seeking a PRN Certified Occupational Therapy Assistant (COTA) to support high-quality OT services in a skilled nursing facility.

The COTA will implement treatment plans, monitor resident progress, and provide direct patient care under OT supervision. Collaboration with nursing, PT, SLP, and other staff will be essential to deliver patient-centered care and maintain accurate EMR documentation.

Qualifications

• Associate’s degree in an OTA program from an accredited school.
• NBCOT certification is required.
• Valid MO OTA license is required.
• SNF or long-term care experience preferred.

Responsibilities

• Implement treatment plans created by the OT.
• Provide direct patient care in the SNF setting.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other staff.
• Maintain accurate EMR documentation.
• Support care planning and discharge planning.
• Ensure HIPAA and CMS compliance.

Position Summary

As a COTA, you will:

• Support delivery of high‑quality OT services
• Help residents regain independence and improve quality of life
• Provide therapeutic interventions under OT supervision

Key Responsibilities

• Implement treatment plans created by the Occupational Therapist
• Provide direct patient care per state and facility guidelines
• Monitor and document resident progress; report changes to the OT
• Educate residents and caregivers on adaptive techniques and equipment
• Collaborate with nursing, PT, SLP, and other healthcare staff
• Maintain accurate, timely documentation in the EMR
• Participate in care planning and discharge planning
• Ensure compliance with HIPAA, CMS, and all regulations

Qualifications

• Associate’s degree from an accredited OTA program
• Current NBCOT certification
• Valid state license as an Occupational Therapy Assistant
• SNF or long‑term care experience preferred
• Strong communication and interpersonal skills
• Able to work independently and as part of a team
