Job summary

Creative Solutions In HealthCare is seeking a compassionate Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. You'll deliver person-centered OT services to residents on a PRN basis, under the supervision of an Occupational Therapist, and help patients improve independence and quality of life.

Responsibilities include administering treatments per the plan of care, documenting thoroughly, and collaborating with the OT, nursing staff, residents, and families.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• Certified Occupational Therapy Assistant (COTA) credential required.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Provide skilled occupational therapy services and interventions in accordance with physician orders under the supervision of an Occupational Therapist
• Respond to requests for service by relaying information and referrals to the Occupational Therapist
• Ensure all treatments are delivered in accordance with the established plan of care
• Provide clinical support and instruction to patients, families, and caregivers
• Monitor patient response to treatment interventions and report changes as appropriate
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations
• Attend required meetings as designated by the Director of Rehab

In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their independence and quality of life.

Your Impact as an Occupational Therapy Assistant (OTA)
