Job summary

Creative Solutions In HealthCare is seeking a Per Diem (PRN) Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will provide person-centered OT services in a skilled nursing setting on a PRN basis, under OT supervision, and support residents to improve independence and quality of life.

Applicants must hold a Texas OTA license and COTA credential. SNF experience is preferred, with strong teamwork and communication skills to coordinate care with the rehab team.

Qualifications

• Active Texas OTA license in good standing.
• COTA credential required for the role.
• Experience in SNF or long-term care settings preferred.
• Knowledge of occupational therapy treatment principles and practices.

Responsibilities

• Plan and administer occupational therapy treatments to residents to improve function.
• Provide skilled OT services under supervision of an OT according to physician orders.
• Respond to service requests by relaying information to the OT.
• Ensure treatments align with the established plan of care.
• Provide guidance to patients, families, and caregivers.
• Monitor patient progress and report changes promptly.
• Complete required documentation in line with policy and regulations.
• Attend meetings as designated by the Director of Rehab.

We are seeking a compassionate and motivated Occupational Therapy Assistant (OTA) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their independence and quality of life.

Your Impact as an Occupational Therapy Assistant (OTA)

• Plan and administer occupational therapy treatments to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Provide skilled occupational therapy services and interventions in accordance with physician orders under the supervision of an Occupational Therapist
• Respond to requests for service by relaying information and referrals to the Occupational Therapist
• Ensure all treatments are delivered in accordance with the established plan of care
• Provide clinical support and instruction to patients, families, and caregivers
• Monitor patient response to treatment interventions and report changes as appropriate
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Occupational Therapy Assistant license
• Certified Occupational Therapy Assistant (COTA) credential required
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication and teamwork skills
