Job summary

Evolve Therapy Services, LLC in Meyersdale, PA is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis. The role plans, organizes, and directs OT services to ensure high-quality resident care in compliance with federal, state, and local standards.

You will screen residents, implement treatment plans under supervision, communicate findings to the team and families, schedule therapy, document and bill services, and perform duties to maintain

Qualifications

• Must have an Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each patient.
• Perform treatment plans for residents; follow short- and long-term goals as established by the plan of care under supervision.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and required to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation as indicated.

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:
