Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility in Clarion, PA. This PRN position supports a partner facility with compassionate, resident-focused care.

You will plan, implement, and monitor OT interventions, conduct screenings, and coordinate with the therapy team to maintain high-quality resident outcomes. Active OTA licensure and an Associate's degree are required, with ongoing CEUs encouraged.

Qualifications

• Requires an Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans per established plan of care.
• Interpret and communicate evaluation findings to residents and families.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing.
• Comply with CMS documentation standards daily.

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy servicesPerform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
