Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to provide in-house skilled nursing facility services on a PRN basis. You will plan, organize, direct OT services in the facility, ensuring compliance with standards and resident care quality.

Responsibilities include screening residents, implementing treatment plans, communicating findings to residents and the care team, scheduling therapy, documenting and billing per plan of care, and maintaining CMS documentation

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Perform treatment plan following established plan of care and supervise therapists' direction.
• Interpret and communicate evaluation findings, plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with the team.
• Document and bill occupational therapy services per plan of care.
• Comply with CMS documentation standards of practice daily.

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

PRN

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
