Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN role supports therapy services under supervision to ensure residents receive high-quality care.

The OTA must hold an active license in the state and participate in continuing education to maintain certification. The role emphasizes compassionate patient care and accurate documentation.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and the team.
• Establish resident therapy schedules with team members.
• Document and bill occupational therapy services per plan of care.
• Perform other duties as assigned to treat patients.
• Comply with CMS documentation standards of practice daily.

Education

Associate's degree in Occupational Therapy

OTA licensure (active in state(s) of practice)
