Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility in Burlington, WI. This PRN position supports patient care and therapy services on-site, aligning with CMS guidelines and facility goals.

Ideal candidates hold an Associate's degree in Occupational Therapy, active OTA licensure in their practicing states, and commit to ongoing professional development.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure required in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plan to residents and follow short- and long-term goals.
• Interpret and communicate evaluation findings, plans and recommendations to residents, families, and team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing.
• Perform all other duties to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Ongoing continuing education and professional development to maintain licensure and certification

Job Position

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
