Job summary

Evolve Therapy Services, LLC in Olivia, MN seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. The position offers a $5,000 sign-on bonus plus tuition reimbursement, with part-time and PRN options available.

As part of a therapy management team, you will plan, organize, and direct occupational therapy services, ensuring high-quality resident care and compliance with CMS guidelines.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings, plans, and recommendations to residents and care teams.
• Establish individual therapy schedules with team members.
• Conduct therapy according to plan and ensure proper documentation and billing of occupational therapy services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type

Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time or PRN positions available

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated
