Job summary

Evolve Therapy Services, LLC is seeking a compassionate Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Wilson, North Carolina. This PRN position focuses on delivering high‑quality OT services and collaborating with the facility team to meet residents’ needs.

The COTA will implement treatment plans, document progress, and ensure compliance with CMS standards while maintaining excellent patient care and communication with families and staff.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Perform treatment plans following goals per plan of care by supervising therapist.
• Interpret and communicate evaluation findings and recommendations to residents, families, and interdisciplinary team members.
• Establish individual therapy schedules with the team.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Perform other duties as required to effectively treat patients.
• Ensure CMS documentation standards are followed daily with point-of-care documentation.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
