Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Lawrenceville, NJ. This PRN role focuses on planning, organizing, and directing OT services to ensure high-quality resident care.

Responsibilities include screening residents, implementing treatment plans, communicating with families and staff, establishing therapy schedules, and maintaining CMS-compliant documentation and billing. Active OTA licensure is required.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine appropriate interventions based on scope of practice.
• Implement treatment plans and follow short- and long-term goals per plan of care.
• Communicate evaluation findings and treatment plans to residents, families, and team.
• Create individual therapy schedules with the care team.
• Deliver therapy per plan and document and bill occupational therapy services.
• Perform additional duties to ensure quality patient treatment.
• Adhere to CMS documentation standards daily with point-of-care records.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
