Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Durham, NC. The OTA will plan, implement, and direct therapy services in accordance with federal, state, and local standards, ensuring high-quality resident care.

Responsibilities include screening, treatment planning, coordination with the team, documentation, and adherence to CMS standards. Ongoing continuing education and licensure maintenance are required.

Qualifications

• Must hold an Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine interventions per scope of practice.
• Perform treatment plans following the established plan of care.
• Communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Establish individual therapy schedules with the team.
• Document and bill occupational therapy services according to plan.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
