Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Butler, Alabama. This PRN role supports ongoing therapy services, ensuring high-quality resident care and compliance with federal/state standards.

Responsibilities include screening residents, implementing treatment plans, documenting progress, and coordinating therapy schedules with the care team.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to identify best intervention based on scope of practice.
• Implement treatment plans and monitor short- and long-term goals under supervision.
• Communicate evaluation findings and recommendations to residents, families, and team.
• Create individual therapy schedules with team coordination.
• Provide therapy per plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Adhere to CMS documentation standards daily with required documentation.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
