Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to join an in-house Skilled Nursing Facility in Dyersburg, TN. This PRN position focuses on planning, directing, and delivering occupational therapy services in alignment with guidelines and resident care goals.

The OTA will screen residents, implement treatment plans, document and bill services, communicate findings to the team and families, and work with the interdisciplinary staff to schedule therapy sessions and

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine intervention or treatment per scope of practice.
• Perform treatment plans for residents; follow goals based on the plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice

• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:
