Job summary

Evolve Therapy Services seeks a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Central City, NE. You will plan, direct, and provide OT services to residents.

Requirements include an Associate’s degree in Occupational Therapy and an active OTA license. Full-time, part-time, or PRN options are available with competitive pay and growth opportunities.

Qualifications

• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to identify best intervention per scope of practice.
• Implement treatment plans and monitor short-/long-term goals.
• Interpret and communicate evaluation findings to residents and families.
• Establish resident therapy schedules with the team.
• Deliver therapy per the plan and document/bill OT services.
• Perform additional duties to effectively treat patients.
• Maintain CMS documentation standards with daily notes.

Job Position: Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To plan, organize, develop, and director Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice

• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy

• MUST have active OTA licensure in state(s) of practice

• Ongoing continuing education and professional development to maintain current licensure and certification
