Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Humboldt, Tennessee. This PRN position supports the facility's occupational therapy services under supervision and follows CMS standards.

Responsibilities include screening residents, implementing treatment plans, communicating findings to families and the interdisciplinary team, scheduling therapy, documenting and billing, and adhering to licensure and facility guidelines.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine the best intervention for each patient.
• Perform treatment plans and follow short- and long-term goals as directed by supervising therapist.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with team members.
• Perform therapy according to treatment plans and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards of practice daily with point of care documentation.

Education

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:
