Job summary

Evolve-Therapy-Services in Antioch, TN, seeks an Occupational Therapist to join an in-house skilled nursing facility. The role covers planning, directing OT services, and ensuring high-quality resident care in compliance with applicable standards.

Responsibilities include screening residents, evaluating needs, developing goals, coordinating with families and the interdisciplinary team, scheduling therapy, and maintaining proper documentation and billing.

Qualifications

• Active Occupational Therapy license in Tennessee (or practicing state)
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Screen residents to determine best interventions.
• Evaluate residents to inform and develop treatment plans with goals.
• Communicate evaluation findings and treatment plans to residents, families, and team members.
• Establish resident therapy schedules with the team.
• Provide therapy per plan and document/bill accordingly.
• Perform other duties as needed to treat patients.
• Oversee COTA and ensure documentation and reimbursement practices meet standards.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
