Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. This position supports full time, part time, or PRN schedules, focusing on delivering quality OT services within a licensed facility.

The role requires an active OTA license in the state(s) of practice and ongoing professional development. You will conduct screenings, implement treatment plans, communicate with residents and families, coordinate therapy

Qualifications

• Occupational Therapy Assistant degree required.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment.
• Implement treatment plans and follow short- and long-term goals.
• Communicate evaluation findings, plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy according to the treatment plan and ensure documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type

Full Time, Part Time, or PRN

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
