Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. This role supports full-time, part-time, or PRN positions at an affiliate facility.

Responsibilities include screening residents, implementing treatment plans, and coordinating with an interdisciplinary team under CMS standards. An OTA license in the state(s) of practice is required and ongoing continuing education is expected.

Qualifications

• Must hold active OTA licensure in state(s) of practice.
• Associate degree in Occupational Therapy required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine intervention or treatment needs.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish individual therapy schedules with team members.
• Conduct therapy per treatment plan and document and bill OT services.
• Perform other duties as assigned and ensure CMS documentation standards are met.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients. Comply with all CMS documentation standards of practice daily with point of care documentations as indicated.

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
