Job summary

Evolve Therapy Services, LLC in Silvis, IL, is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility.

This full-time role involves planning, organizing, and directing OT services, conducting resident screenings, developing and following treatment plans, and communicating results to residents, families, and the interdisciplinary team. You must maintain active IL OTA licensure and engage in ongoing education to support CMS standards and proper billing.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA license in IL (or state of practice).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine best intervention per scope of practice.
• Implement treatment plans; follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules in collaboration with team members.
• Deliver therapy according to the treatment plan and ensure proper documentation and billing of OT services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
