Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to support an in-house skilled nursing facility in Lake Geneva, WI. The role involves planning, directing, and delivering occupational therapy services to residents in accordance with applicable standards.

Responsibilities include conducting screenings, developing treatment plans, coordinating with the interdisciplinary team, and ensuring proper documentation and billing under CMS guidelines.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST hold active OTA licensure in state(s) of practice.
• Ongoing continuing education and licensure maintenance.

Responsibilities

• Screen residents to determine best intervention based on scope of practice.
• Develop and implement treatment plans aligned with goals.
• Communicate evaluation findings and recommendations to residents, families, and the care team.
• Schedule therapy sessions with colleagues to coordinate care.
• Deliver therapy per plan and document care and billing accurately.
• Fulfill additional duties to ensure quality patient care.
• Maintain CMS documentation standards in daily charting.

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
